What to expect when you walk in
Arrive
Come to 725 Park Ave, Bridgeport during clinic hours (Mon–Fri, 9 AM–4 PM). No appointment necessary.
Check In
Our front desk team will get you registered. Bring an ID if you have one — but it’s not required.
See Your Provider
You’ll meet with a practitioner for your visit. Primary care, behavioral health, or both.
Follow Up
If you need follow-up care, prescriptions, or referrals, we handle it. We stay with you.
